GroupTerm Life and Accidental Death & Dismemberment

Available to groups with one or more covered employees
For groups of 1 to 19 the minimum benefit is $15,000, maximum benefit is $50,000. Guarantee issue is $30,000.
For groups of 20+ employees there is no minimum benefit. Guarantee Issue amounts exceed $30,000.
All schedules available: flat, % of salary, class.

Short Term Disability

Available to groups with two or more covered employees
Minimum benefit is $40, maximum benefit is $500; flat amounts or % of salary by class
Benefit Plans available: 1st day accident, 8th day sickness, 26 week maximum 1st day accident, 8th day sickness, 13 week maximum

Long Term Disability

For groups of two or more employee
Guaranteed issue for groups of 6 or more
All industries are eligible
Can pay to age 65
